Referrals
Perspectives Behavioral Health Management, LLC, strives to make the referral process as accessible and streamlined as possible. We have gone to great lengths to minimize the amount of time and paperwork typically associated with the referral process.
Referrals can be made for an evaluation by a clinical professional to determine the appropriateness for the program or any of its services. Individuals diagnosed with a psychological disturbance that meets DSM-IV criteria and are determined to be appropriate for services will be seen and supervised by an attending psychiatrist.

be handled in a timely and efficient manner.

Patient Rights Under HIPAA
A BRIEF OVERVIEW
Right to A Notice of Privacy Practices
Patients must be given a copy of our Notice of Privacy Practices. We
must post it at each service location and provide it to patients upon
request.
Right to Access Information
Patients may inspect and get a copy of their protected health information,
subject to certain limitations. We may require patients to make requests
in writing and charge a reasonable fee for copying.
Right to Request Amendments
Patients may ask us to amend their protected health information. We
may require that patients put the requests in writing. We are not required
to approve or grant all requests.
Right to Request Alternative Means of Communication
Patients may ask us to communicate their protected health information
by alternative means (such as by fax) or to an alternative location
(such as a business address). We are not required to approve all requests.
Right to Request Restrictions
Patients may ask us to restrict how we use or disclose their protected
health information. We may require requests to be made in writing and
are not required to grant all restrictions
Right to An Accounting
Patients who request it must be given an accounting of disclosures we
have made of their protected health information. Some restrictions do
apply.
Right to Complain About Our Privacy Practices
Patients may make complaints about our privacy practices to our Privacy
Officer and the U.S. Department of Health and Human Services if they
believe we are not complying with HIPAA.
